Notice of Member Rights and Responsibilities
As a Member of our community, you are entitled to certain Rights. These Rights are summarized below:
Your Rights:
Your Have the Rights to:
- Privacy and confidentiality
- Dignity and respect
- A safe and secure environment for the delivery of your care
- Request a copy of your paper or electronic medical record
- Receive information about your illness, prognosis and care plan
- Make decisions regarding your care, refuse or suspend care
- Know the identity and qualifications of the staff members involved in your care
- Nominate someone to act on your behalf, in accordance with legal requirements
- File a complaint if you believe your privacy rights have been violated
By joining our community, you are also bound by our organization’s Code of Conduct, and, as
such, have the following Responsibilities:

- Following the rules, policies and regulations of the Facility
- Treating members of our staff and community with dignity and respect
- Providing complete and truthful information about your health and medical history and reporting any changes in your health to your care team
- Understanding all aspects of your care plan and asking for clarifications if needed
- Complying to treatment protocols as advised by your care team
- Bearing the financial expenses of your membership and complying with the Payment and Billing policies of the Facility
Zero Tolerance Policy
The staff of the Company have a right to care for others without fear of being attacked or abused. The Company has a Zero Tolerance Policy if a member of its staff is treated in an abusive or violent way. Our staff aims to be polite, helpful, and sensitive to all Members’ individual needs and circumstances. However, aggressive behaviour, be it violent or abusive, will not be tolerated and may result in you being removed from the premises/community, or in extreme cases, the relevant law enforcement authorities being contacted. The types of behaviour that would be found unacceptable and in breach of the Company’s Code of Conduct include, but are not limited to:- Using bad language or swearing at staff
- Any physical violence towards staff or other Members, such as pushing or shoving
- Verbal abuse towards the staff in any form including verbally insulting the staff
- Racial abuse and sexual harassment
- Causing damage/stealing from the Company’s premises, staff or Members
